Frequently Asked Questions

What do I need to bring with me?

You should have:​

  • Your therapy prescription from your doctor (if you have one)

  • Your driver's license

  • Your insurance card(s)

  • A list of your current medications

  • Any medical imaging you've had recently (related to your reason for therapy)

Do you accept my insurance?

We accept most major insurances, but we would be happy to call your insurance company and verify what, if any, benefits you have for physical or occupational therapy.  Call one of our offices and speak with a front desk associate!

What happens during my first visit?

For your first visit:

  • Arrive 15-20 minutes before your scheduled appointment to fill out paperwork and to answer our medical history questionnaire.

  • Our front desk manager will make copies of your therapy prescription, insurance card(s), ID, any imaging results, and list of medicines.

  • You will then be seen by one of our therapists for your initial evaluation, where you will discuss with them your medical history, reason for your visit, and what your PT/OT plan might look like going forward.  The therapist may perform some simple tests to determine your movement limitations; this will help them determine the best course of action for your therapy.

What should I wear?

We usually recommend that patients wear loose-fitting exercise clothing and sneakers for maximum comfort.
